      摘要 目的:对肩峰肱骨距离、冈上肌肌腱厚度以及肩、颈姿势进行评估,对肩峰下撞击综合征(SIS)患者肩峰下空间变化的机制进行探讨。 方法:纳入试验组SIS患者20例,同期招募的年龄、性别、体重均匹配健康人对照组20例(试验组及对照组均为右利手)。使用超声测量肩峰肱骨距离(AHD)以及冈上肌肌腱厚度。使用GPS姿势分析系统分析头前伸姿势(FHP)和肩前伸姿势(FSP)。分别测量颅椎角(CA)和肩前伸角度(FSA)联合上述两种方式对肩峰下空间变化的机制进行探讨。 结果:试验组冈上肌肌腱厚度明显大于对照组,两者差异具有显著性意义(P<0.01)。两组之间AHD无显著性差异(P>0.05)。试验组冈上肌厚度与AHD比值大于对照组,差异具有显著性意义(P<0.01)。试验组FSA小于对照组,具有显著性差异(P<0.01)。两组CA相比,差异无显著性意义(P>0.05)。试验组FSA与AHD两者间存在中度相关(P<0.05,R=0.62)。CA与AHD之间不存在相关关系(P>0.05)。 结论:SIS与试验组患者相比,冈上肌肌腱更厚且肩前伸更为明显。

Preliminary study on ultrasound and posture analysis of subacromial space in patients with subacromial impingement syndrome    Download Fulltext

Abstract:
      Abstract Objective: To explore the mechanism of subacromial space changes by evaluating acromiohumeral distance (AHD), supraspinatus tendon thickness and shoulder-neck posture in patients with subacromial impingement syndrome. Method: Twenty patients with subacromial impingement syndrome were assigned to observation group. 20 healthy person matched observation group in age, gender and weight were assigned to control group. AHD and supraspinatus thickness in both groups were measured by using ultrasound and shoulder-neck posture was evaluated with global posture system (GPS) by measuring forward shoulder angle (FSA) and craniovertebral angle (CA). Statistic difference was assigned at P<0.05. Result: The observation group had a significantly thicker tendon(P<0.01) and a greater tendon occupation ratio(P<0.01) compared with matched controls. There was no difference in AHD between two groups(P>0.05). The observation group had a significantly smaller forward shoulder angle(P<0.01). There was no difference in craniovertebral angle between two groups(P>0.05). There was a moderate correlation between AHD and FSA in observation group(P<0.05,R=0.62),but no correlation between AHD and CA in observation group(P>0.05). Conclusion: The supraspinatus tendon was thicker and occupied a greater percentage of AHD, supporting an intrinsic mechanism. The significantly smaller forward shoulder angle and moderate relationship between AHD and FSA in observation group supported an extrinsic mechanism.
