      摘要 目的:探讨计算机运动反馈训练对脑卒中偏瘫患者手功能康复的疗效。 方法:采用随机数字法将46例脑卒中患者分为对照组(n=23)和试验组(n=23)。在接受基础常规康复训练的基础上,试验组采用计算机运动反馈训练治疗,对照组采用常规手功能康复治疗,每天1次,每次30min,每周5次,共4周。治疗前后采集患侧手部的表面肌电值,同时采用Fugl-Meyer评定量表(FMA)、改良Ashworth分级评价量表(MAS)和改良Barthel指数(MBI)来评估患者的功能。 结果:共43例患者完成试验。治疗后,对照组FMA、MBI评分以及腕屈肌、腕伸肌、指伸肌、拇短展肌表面肌电积分肌电值(iEMG)、均方根值(RMS)均较治疗前提高(P<0.05);试验组FMA、MAS、MBI评分以及腕屈肌、腕伸肌、指伸肌、拇短展肌表面肌电iEMG、RMS均较治疗前提高(P<0.05);治疗后试验组的FMA总分及手部分评分、MBI评分、腕伸肌RMS、指伸肌iEMG改善优于对照组(P<0.05)。 结论:计算机运动反馈训练可有效促进脑卒中患者手功能的康复及日常生活活动能力的恢复。

Abstract:
      Abstract Objective: To study effects of the computer-assisted motion feedback training on the hand dysfunction in patients after stroke. Method: Forty-six subjects, suffering from stroke, were randomly assigned to control group (n=23) and trial group (n=23). Based on the basic rehabilitation, the trial group accepted computer-assisted motion feedback training, while the control group accepted routine rehabilitation of hand function. All participants received treatment for 4 weeks (30min once daily,5 days/week). Before and after training, the surface electromyography data were collected and the wrist/hand subscales of the Fugl-Meyer assessment(FMA), modified Ashworth scale(MAS), modified Barthel index (MBI)were measured to assess the function of patients. Result: A total of 43 patients completed the experiment. The FMA score, MBI score,the root mean square (RMS) and integrated electromyography(iEMG)of the WF, WE, ED, APB, the FMA score, MAS score, MBI score, improved in both group after training(P<0.05). But the total score of FMA, hand Subscales of FMA score, MBI score, RMS of WE and iEMG of ED of the trail group were higher than those of the control Group after training (P<0.05). Conclusion: Computer-assisted motion feedback training is effective for improvements of the hand function and activities of daily living in patients after stroke.
