      摘要 目的:前交叉韧带重建术(anterior cruciate ligament reconstruction, ACLR)是前交叉韧带损伤的常见手术,患者术后重返运动情况与不同身体功能的关系未有定论。本研究旨在明确高运动需求者ACLR术后不同时期的肌肉力量、本体感觉和足底触觉与重返运动之间的关系,以期为ACLR精准康复方案的制订提供依据。 方法:本研究共招募了42例ACLR术后6个月以上的高运动需求者,根据术后时间,将20例纳入6—11个月组,22例纳入12—18个月组。测试了2组患者的肌肉力量、本体感觉和足底触觉,以及重返运动(Knee Santy Athletic Return to Sports, K-STARTS)总分,其包含心理得分(ACL Return to Sports after Injury scale, ACL-RSI)和功能得分。使用偏相关分析探索了指标间的相关性。 结果:2组患者屈膝力量与K-STARTS总分和ACL-RSI得分显著相关,伸膝力量与K-STARTS总分和功能得分显著相关;45°的本体感觉与K-STARTS总分和功能得分显著相关;6—11个月组患者的第五跖骨的足底触觉与K-STARTS总分和功能得分显著相关,而在12—18个月组患者中其仅与功能得分显著相关。 结论:高运动需求者ACLR术后的肌肉力量和本体感觉与K-STARTS总分、ACL-RSI得分和功能得分有关,足底触觉仅在术后6—11个月与K-STARTS总分有关,在术后12—18个月与K-STARTS总分无关。

Correlations of muscle strength, proprioception and plantar tactile sensation to return-to-sport readiness in patients with high sports demands at different periods after anterior cruciate ligament reconstruction    Download Fulltext

      Abstract Objective: Anterior cruciate ligament reconstruction (ACLR) is the most common surgery for ACL injuries, and the relationships between patients' return to sports (RTS) and different physical functions are inconclusive. This study aimed to investigate the correlations between muscle strength, proprioception,plantar tactile sensation and return to sport readiness at different periods after ACLR among individuals with high sports demands, to provide a basis for the development of accurate rehabilitation programs after ACLR. Method: A total of 42 participants with high sport demands and received ACLR for at least 6 months were enrolled in this study, and 20 of them were included in the 6—11 month and 22 in the 12—18 month groups according to the postoperative time. The muscle strength, proprioception and cutaneous sensation were tested. RTS readiness was measured with the Knee Santy Athletic Return to Sport (K-STARTS) test, which consists of a psychological scale (Anterior Cruciate Ligament Return to Sports after Injury scale [ACL-RSI]) and seven functional tests. Partial correlations were used to determine their correlations. Result: The knee flexion torque was significantly correlated with K-STARTS total and ACL-RSI scores, and knee extension torque was significantly correlated with K-STARTS total and functional scores; Proprioception at 45° was significantly correlated with K-STARTS total and functional scores. In the 6—11 month group, cutaneous sensation of the fifth metatarsal head was significantly correlated with the K-STARTS total and functional scores, while in the 12—18 month group it was significantly correlated with functional score. Conclusion: The muscle strength and proprioception in high sport demands after ACLR were correlated with K-STARTS total score, ACL-RSI score and functional score, and cutaneous sensation was only associated with K-STARTS total score at 6-11 months postoperatively and not with K-STARTS total scores at 12-18 months after ACLR.
