| 目的：探讨“Any Two”试验在卒中后误吸筛查中的应用价值。方法：81例急性脑卒中患者接受“Any Two”试验和纤维鼻咽喉镜吞咽功能检查(FEES)。以FEES检查为金标准，计算“Any Two”试验对于急性卒中患者误吸诊断的灵敏度、特异度、阳性预测值和阴性预测值。对比“Any Two”试验阳性患者和阴性患者的肺炎发生率。结果：“Any Two”试验对卒中后误吸诊断的灵敏度为92.5%，特异度为31.7%，阳性预测值为56.9%，阴性预测值为81.3%。18例无症状性误吸患者中有17例“Any Two”试验为阳性。“Any Two”试验阳性的患者肺炎发生率为15.4%，试验阴性的患者均未发生肺炎，但两组间肺炎发生率的差异在统计学上未见显著性意义(P＝0.094)。结论：“Any Two”试验在卒中后误吸的评估中是一个有价值的筛查工具。 |

| Objective: To investigate the value of “Any Two” test in screening for misaspiration after stroke. Method: Eighty-one patients with acute stroke were given both “Any Two” test and Fiberoptic Endoscopic Examination of Swallowing(FEES). “Any Two” test was measured against the FEES study for sensitivity, specificity, positive predictive value and negative predictive value for misaspiration in acute stroke patients. The relation between the result of “Any Two” test and pneumonia post-stroke was analyzed. Result: Sensitivity, specificity, positive predictive value and negative predictive value of “Any Two” test for misaspiration were 92.5%,31.7%,56.9% and 81.3%，respectively. Of 18 patients with silent aspiration, 17 patients showed positive on “Any Two” test. The incidence of pneumonia in patients who were positive on “Any Two” test was 15.4%. No patients who were negative on “Any Two” test developed pneumonia during addition. But there was no significant difference between these two groups. Conclusion: “Any Two” test is a valid and valuable screening tool for misaspiration in acute stroke patients. |
