目的:通过系统分析不同认知水平及各类型的认知障碍对脑卒中患者平衡功能的影响,探讨认知功能在平衡调节中所起的作用。
方法:对72例年龄在39—80岁的脑卒中患者分别用LOTCA和BBS、PASS及Brunnstrom运动功能分期等量表分别对其认知功能、平衡功能和肢体运动功能进行评定。在排除下肢运动功能对平衡功能的影响后,对患者平衡功能与认知功能进行偏相关分析。
结果: BBS和PASS均与LOTCA高度偏相关性(P<0.01)。“瘫痪侧站立”与LOTCA总分、“注意力”、“视知觉”、“空间知觉”、“动作运用”的相关性比“非瘫痪侧站立”与它们的相关性高。“姿势变换”与“视觉运动组织”、“动作运用”、“空间知觉”、“视知觉”、“注意力”、“定向”的偏相关系数大于“姿势维持”与其偏相关系数。 “360°转身”与LOTCA各分量表均有较高的相关性(P<0.05)。
结论:视运动组织、空间知觉、视知觉和注意力等认知因素是脑卒中患者的平衡过程的重要环节,对患者的平衡功能有不可忽视的影响。运动调节过程越复杂越受到认知成分的限制。

Abstract
Objective: To analyze the relationship between cognition function and balance function in hemiplegic patients post-stroke, and to evaluate the contribution of cognition factors in balance course.
Method: Seventy-two hemiplegic subjects post-stroke were recruited and evaluated with the following assessment: Loewenstein occupational therapy cognition assessment battery(LOTCA) for cognition function; Berg balance scale(BBS) and postural assessment scale for stroke patients(PASS) for balance function; Brunnstrom stage for limbs′ movement function. Correlational analysis quantified the relationship between balance function and cognition function in all patients recruited.
Result: Middle to high partial correlations were found between balance function and cognition function by controlling Brunnstrom stage of lower limbs. The partial correlationship between balance function and visuomotor organization, spatial perception, visual perception were high. The correlation between balance function and cognition function were stronger when the subjects stood with weak legs than those with sound legs. And those were stronger in postural maintain than postural shifting.
Conclusion: There are significant correlation between balance function and cognition function in hemiplegic patients with stroke, and the visuomotor organization, spatial perception, visual perception and attention are the most important cognition factors which influence and participate in balance control post-stroke. The influences of motor praxis, orientation and thinking operations to balance are middling.The more complex the motor control, the more limited by cognition. |
