      摘要 目的:观察不同强度有氧运动对2型糖尿病患者血糖、血脂、体重、身体成分及最大摄氧量的影响。 方法:45例2型糖尿病患者随机分为高强度组(75%VO2max)(n=15)、中强度组(50%VO2max)(n=15)及对照组(n=15)。高强度及中强度组均进行为期12周的有氧运动,每周3次。高强度组每次训练2组,每组20min,组间休息5min,共45min,中强度组每次持续运动55min。所有患者训练前后均进行空腹血糖(FPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、体重(body weight)、脂肪重量(FW)、瘦体重(LBM)、最大摄氧量(VO2max)测定;测定单次运动的能量消耗和整个运动过程的总能量消耗。 结果:试验前3组各指标均无明显差异(P>0.05);试验后高强度和中强度组所有指标均较对照组有明显差异(P<0.05),高强度组与中强度组相比仅最大摄氧量有显著性差异(P<0.05);单次运动能量消耗及12周总运动能量消耗比较,中强度组与高强度组无显著性差异(P>0.05)。 结论:在运动量没有差别的情况下,中强度及高强度有氧运动在改善2型糖尿病患者血糖、血脂、体重、身体成分方面没有显著性差异,但高强度有氧运动更能明显提高2型糖尿病患者的心肺耐力。

      Abstract Objective: To observe the effects of aerobic exercises in different intensities to the glucose, lipids, body weight, body composition and VO2max of type 2 diabetes patients. Method: Forty-five patients with type 2 diabetes were divided into high-intensity (75%VO2max) group (n=15), moderate-intensity (50%VO2max) group (n=15) and control group (n=15). Aerobic exercise duration was 12 weeks, 3 times every week. High-intensity group did exercise for 45min per time with 2 sections, and each section lasted 20min with 5min interval, moderate-intensity group administered 55min continuously exercise every time. All patients were evaluated with FPG, HbA1c, TC, TG, LDL, body weight, fat mass, lean body mass and VO2max before and after training; both high-intensity exercises group and moderate-intensity exercise group were assessed energy expenditure of a single bout exercise and the whole process of movement. Result: There was no significant difference (P>0.05) in all variables in three groups before training. After training, the FPG, HbA1c, TC, TG, LDL, body weight, fat mass, lean body mass and VO2max of high-intensity group and moderate-intensity were significantly different with control group (P<0.05), only VO2max had significant difference between high-intensity group and moderate-intensity group(P<0.05). There was no significant difference (P>0.05) in the energy expenditure of a single bout exercise and the whole process of movement between the high-intensity exercises group and moderate-intensity exercise group(P>0.05). Conclusion: When the energy expenditure had no difference, there was no significant difference in improving the glucose, lipids, body weight and body composition of type 2 diabetes patients between moderate-intensity aerobic exercise and high-intensity aerobic exercise. However, high-intensity aerobic exercise was more effective to augment cardio-pulmo nary endurance.
