      摘要 目的:研究呼吸训练配合常规康复训练对偏瘫患者肺功能、运动功能、ADL能力及生存质量的影响。 方法:将58例脑卒中后偏瘫的患者,分为训练组(n=33)和对照组(n=25)。对照组进行常规康复训练;训练组在常规康复训练基础上配合呼吸训练,在训练前后均进行肺功能及血氧饱和度检测,包括用力肺活量(FVC,第一秒呼气流速(FEV1/FVC),呼气峰值流速(PEF),动脉氧分压(PaO2)、动脉血氧饱和度(SpO2);简式Fugl-Meyer运动功能(FMA)评定、Barthel指数(MBI)ADL能力(ADL)评定、健康调查简表(the MOS item short form health survey,SF-36)生存质量评定。 结果:经过12周的训练后,两组患者的肺功能(FVC、FEV1/FVC、PEF)、血氧指标(PaO2、SpO2)、简式FMA评分、MBI、SF-36评分较治疗前均得到明显改善(P<0.05),训练组明显优于对照组(P<0.01)。 结论:呼吸训练联合常规康复训练可改善偏瘫患者的肺功能、运动功能、ADL能力以及生存质量,可作为有效的干预手段应用于临床。

Abstract:
      Abstract Objective: To study the effect of respiratory training and conventional rehabilitation on pulmonary function, motor function, activity of daily life and quality of life in hemiplegic patients. Method: A total of 58 patients with post-stroke hemiplegia were divided into training group(n=33) and control group(n=25) by themselves. Both groups received conventional rehabilitation. Training group received respiratory training in addition. Tests were taken including Pulmonary function and blood gas analysis , include forced vital capacity(FVC), percentage of forced expiratory volume in first second to forced vital capacity(FEV1/FVC), peak expiratory flow(PEF), arterial partial pressure of oxygen(PaO2), and oxygen saturation(SpO2) were taken before and after training. Fugl-Meyer Assessment (FMA), Modified Barthel Index (MBI), MOS item short form health survey (SF-36) scales and quality of life were analyzed also. Result:After 12 weeks interventions, the above assessments of both groups significantly improved than that before training(P<0.05), and , training group were better than control group (P<0.01). Conclusion:The combinations of respiratory training and conventional rehabilitation could improve the lung function, motor function, quality of life of patients with hemiplegia', and may be used as clinical effective intervention method
