目的:探讨手摇车训练同步结合功能性电刺激治疗对脑卒中后偏瘫患者上肢功能恢复的影响。
方法:将40例脑卒中偏瘫患者,随机分为试验组(n=20)和对照组(n=20)。两组患者均给予常规康复治疗,对照组患者同时接受仅有主被动运动功能手摇车训练,试验组患者在进行手摇车训练的同时辅以具有同步性功能性电刺激治疗。两组均接受4周的治疗。在治疗前、治疗4周后采用上肢Fugl-Meyer运动功能评测上肢部分(FMA-UE)、简易上肢功能评价(STEF)、改良Barthel指数(MBI)及改良Ashworth量表(MAS)对两组患者上肢功能进行评估比较。
结果:治疗4周后,两组患者上肢FMA-UE评分、STEF评分、MBI评分及MAS评分较治疗前均明显改善(均P<0.05);除了MAS评分外,试验组患者以上评分均显著优于对照组水平,差异有显著性意义(均P<0.05)。
结论:手摇车训练同步功能性电刺激能更显著改善脑卒中后患者上肢运动功能,提高患者日常生活自理能力。 |

Abstract
Objective: To explore the influence of resistant hand wheel synchronized with functional electrical stimulation on upper limb function in post-stroke patients.
Method: Forty patients with hemiplegia after stroke were randomly assigned into experiment group (n=20) and control group (n=20). Patients in both groups accepted routine rehabilitation therapy. Patients in the control group also received active-passive resistant hand wheel exercise. Resistant hand wheel exercise synchronized with functional electrical stimulation were introduced in the experiment group. Patients in both groups took 4 weeks therapy. All patients in the 2 groups were assessed with Fugl-Meyer assessment upper extremity(FMA-UE), simple test(STEF) for evaluating hand function, modified Barthel index(MBI) and modified Ashworth scale(MAS) at the beginning and 4 weeks after treatment.
Result: After 4 weeks of treatment, the comparison of FMA-UE score/STEF score/MBI score/MAS score in the 2 groups were better than before, the differences were statistically significant (all P<0.05). The FMA-UE score/STEF score/MBI score in the experiment group were better than the control group (all P<0.05) except the MAS score.
Conclusion: resistant hand wheel exercise synchronized with functional electrical stimulation could significantly improve upper limb function and enhance activity of daily living in stroke patients. |
