      摘要 目的:探究经颅直流电刺激(tDCS)左侧背外侧前额叶改善卒中后患者认知功能的影响,并进一步分析tDCS改善认知功能的可能机制。 方法:将30例卒中后血管性认知功能障碍患者,随机分为试验组(n=15)和对照组(n=15),2组患者均接受常规康复治疗,试验组同步行tDCS真刺激,对照组行tDCS假刺激,治疗时间为4周。在治疗前后对患者统一进行认知神经量表评估(包括MoCA、MMSE及LOTCA量表),比较两组之间认知功能。 结果:治疗前两组患者各项观察指标均无显著性差异(P>0.05),试验组患者的MoCA、MMSE、LOTCA认知评定量表评分较治疗前增加(P<0.05),对照组患者的MMSE、LOTCA认知评定量表评分较治疗前增加(P<0.05)。治疗后试验组和对照组组间相比,MoCA、MMSE、LOTCA认知评定量表差异均有显著性意义(P<0.05)。 结论:tDCS阳极刺激左侧背外侧前额叶有助于改善脑卒中后血管性认知功能障碍患者的认知功能。

Abstract:
      Abstract Objective: To investigate the efficacy of transcranial direct current stimulation (tDCS) over the left dorsolateral prefrontal cortex in improving the cognitive function in vascular cognitive impairment patients after stroke, and further analyse the possible mechanism of tDCS in improving cognitive function. Method: Thirty patients with vascular cognitive impairment after stroke were randomly divided into experimental group (n=15) and the control group (n=15).The patients in both groups received routine rehabilitation therapy, and the test group received tDCS therapy synchronously, while the control group received the sham tDCS stimulation. The treatment lasted for 4 weeks. Before and after treatment, cognitive function was compared between the two groups by unified cognitive neurology scale assessment (including MoCA,MMSE,LOTCA scale). Result: There was no significant difference in the observation indices before treatment between the two groups (P>0.05). MoCA, MMSE, LOTCA cognitive rating scale score were improved after treatment in the experimental group (P<0.05). MMSE, LOTCA cognitive rating scale score were improved after treatment in the control group (P<0.05). After treatment, the MoCA, MMSE, LOTCA cognitive rating scale in the experimental group were significantly better than those in the control group (P<0.05). Conclusion:Anodal tDCS over the left dorsolateral prefrontal cortex can improve the cognitive function in vascular cognitive impairment patients after stroke.
