目的：研究探讨脑性瘫痪（脑瘫）儿童日常生活活动（ADL）康复护理评定量表在脑性瘫痪儿童日常生活活动能力评定中的信度与效度。
方法：选择2017年3月—2017年9月到佳木斯大学附属第三医院住院治疗的痉挛型脑瘫儿童256例，采用脑瘫儿童ADL康复护理评定量表、儿童功能独立性测量量表（Wee FIM）、Gesell婴幼儿发展量表进行评定。ADL量表信度和二级类目分信度检验采用Kappa一致性分析方法；ADL与Wee FIM、Gesell同步效度采用Spearman相关分析。
结果：脑瘫儿童ADL康复护理评定量表9个二级类目的整体Kappa值为0.869，信度优；二级类目分信度中，Kappa值最高项目为床上运动翻身0.914，最低项目为认知交流0.833；评估者间信度为0.702—0.875，信度中到优。脑瘫儿童ADL康复护理评定量表和Wee FIM、Gesll的比较研究得出两者相关性良好，P<0.01。
结论：脑瘫儿童ADL康复护理评定量表具有很高的重测信度，内部一致性较好，与Wee FIM量表、Gesell量表有良好的关联效度。采用ADL康复护理评定量表对脑瘫康复护理日常生活活动能力评定可靠、有效。 |

| Abstract
Objective: To investigate the reliability and validity of the ADL Rehabilitation Nursing Rating Scale for children with cerebral palsy in the evaluation of children's activities of daily life.
Method:A total of 256 children with spastic cerebral palsy who were admitted to the Third Affiliated Hospital of Jiamusi University from March 2017 to September 2017 were enrolled. The cerebral palsy children's daily living activity assessment scale (ADL) and the child function independence measure (Wee FIM) and the Gesell Infant Development Scale (Gesell) were selected for evaluation. The Kappa consistency analysis method was used for ADL scale reliability and secondary category reliability test. Spearman correlation analysis was used for ADL and Wee FIM and Gesell synchronization validity.
Result: The overall kappa value of 9 secondary categories of cerebral palsy children was 0.869, and the reliability was excellent. In the secondary category classification, the highest kappa value was 0.914 for bed movement, and the lowest item was 0.833 for cognition，the inter-evaluator's reliability was 0.702—0.875, and the reliability was excellent. A comparative study of the ADL rehabilitation nursing rating scale and Wee FIM and Gesell of children with cerebral palsy showed that the correlation was good, P<0.01.
Conclusion:The ADL rehabilitation nursing rating scale for children with cerebral palsy has high test-retest reliability and good internal consistency. It has good correlation validity with Wee FIM scale and Gesell scale. It is reliable and effective to use the ADL scale to evaluate the ability of cerebral palsy rehabilitation care activities of daily life. |
