      摘要 目的:观察电针对局灶性脑缺血再灌注模型(middle cerebral artery occlusion model/reperfusion model,MCAO/R)大鼠脑缺血皮质区VEGF、VEGFR-2、CD34表达水平的影响,探讨针刺调控脑缺血皮质区神经血管单元(neurovascular unit,NVU)微血管再生,促进MCAO/R大鼠神经功能重塑的作用机制。 方法:雄性SD大鼠27只随机分为假手术组,模型组,针刺组,每组9只。采用大脑中动脉线栓法制备MCAO/R大鼠。MCAO/R后1d行针刺治疗,取大鼠“百会”穴及“足三里”为针刺穴位,每次刺激时间为20min,每日1次,连续治疗14d。神经功能缺损评分法评价神经功能缺损情况,HE染色观察MCAO/R大鼠脑梗死侧脑组织病理形态变化,免疫组织化学法检测MCAO/R大鼠脑缺血灶周围皮质VEGF、VEGFR-2、CD34的表达。 结果:与假手术组比较,造模组造模后出现神经功能缺损,神经功能缺损评分明显增高(P<0.05);HE染色见明显脑缺血灶;脑缺血灶周围皮质VEGF、VEGFR-2、CD34表达增高(P<0.05)。与模型组比较,针刺组神经功能缺损评分逐渐降低(P<0.05);针刺3、7、14d时,针刺组神经缺损评分低于模型组(P<0.05);电针14d后,针刺组脑缺血灶周围皮质VEGF、VEGFR-2、CD34的表达明显高于模型组(P<0.05)。 结论:电针可以改善MCAO/R大鼠神经功能,其作用可能与提高MCAO/R大鼠缺血灶周围皮质VEGF、VEGFR-2、CD34表达水平,促进脑缺血皮质区NVU微血管再生有关。

      Abstract Objective: To illustrate the effects of acupuncture on the expression of VEGF、VEGFR-2 and CD34 in cerebral ischemic cortex after middle cerebral artery occlusion and reperfusion model(MCAO/R) rats, and study the mechanism of the brain angiogenesis of neurovascular unit(NVU) around the ischemic focus. Method: Twenty-seven healthy adult male SD rats were randomly divided into sham operation group (n=9), model group (n=9), acupuncture group (n=9).The MCAO/R modal was established by occluding the middle cerebral artery. One day after modeling, acupuncture at Baihui, Zusanli 20min daily for successive 14 days. The neurological deficit scores were evaluated on 12h, 1,3,7,14 days after modeling HE staining was used to observe the pathological changes of cerebral tissue in MCAO/R rats. Immunohistochemical method was used to detect the expression of VEGF、VEGFR-2 and CD34 in the ischemic cortex of MCAO/R rats. Result: The neurological deficits index increased significantly at 2h after operation and then decreased gradually at day 1,3,7,14. The decreasement were even more in acupuncture group(P<0.05). The expression of VEGF and VEGFR-2,CD34 of rats increased significantly in model group 14days after operation(P<0.05), but the increase in acupuncture group was much more than in model group (P<0.05) . Conclusion: The acupuncture can promote the recovery of defective neurological function of MCAO/R rats, and increase VEGF、VEGFR-2 and CD34 in the infarct regions, thus increase the brain angiogenesis of NVU around the cerebral ischemic focus.
