| 摘要
目的:基于额叶静息脑电alpha频谱特征探讨低频重复经颅磁刺激(rTMS)联合太极拳训练改善老年睡眠障碍患者的睡眠质量、促进记忆功能的作用。
方法:将符合纳排标准的老年睡眠障碍患者110例,区组随机分为试验组(太极拳训练联合rTMS)和对照组(太极拳训练联合伪rTMS),干预5次/周,连续6周。比较组间干预前后匹兹堡睡眠质量指数(PSQI)、腕式体动仪监测的夜间睡眠特征、韦氏记忆量表水平变化;探讨日间静息额叶alpha频谱功率变化,及与记忆功能的相关性。
结果:103例受试完成干预。干预前,两组受试者各项指标比较均无显著性差异(P>0.05)。干预后,相较于对照组(52例),试验组(51例)睡眠效率、记忆商MQ显著提高(P<0.05);PSQI及额上回、额中回alpha频段功率显著降低(P<0.05);试验组的右额上回FC2电极alpha频段功率与日间功能障碍呈现正相关(r=0.303,P=0.045);记忆商MQ与alpha频段功率呈现正相关,表现在左额上回AF3电极(r=0.306,P=0.034);右额中回F6电极(r=0.300,P=0.038);右额上回FC2电极(r=0.301,P=0.038),右额中回FC4电极(r=0.308,P=0.033)。
结论:rTMS联合太极拳训练显著改善老年睡眠障碍患者的睡眠质量,并通过降低其在维持记忆功能时代偿性增高的额叶alpha频段功率,以促进记忆功能提高。 |

| Abstract
Objective:To explore the effect of low-frequency repetitive transcranial magnetic stimulation (rTMS) combined with Tai Chi Chuan on sleep quality and memory function improvement in elderly patients with sleep disorders based on frontal resting state EEG alpha frequency spectrum characteristics.
Method:Total 110 elderly patients with sleep disorders who met the enrollment criteria were randomly allocated into two groups: the experimental group (Tai Chi Chuan + rTMS) and the control group (Tai Chi Chuan+sham rTMS). The intervention was conducted 5 times per week for 6 weeks. The Pittsburgh sleep quality index (PSQI), sleep characteristics evaluated by wrist-worn activity monitor,Wechsler memory scale, and the changes in daytime resting frontal alpha frequency spectrum power and its correlation with memory function were compared and discussed.
Result:Total 103 patients completed the intervention. There were no significant differences in any indicators between the two groups in baseline(P>0.05). After 6 weeks, compared to the control group(n=52), the experimental group(n=51) showed significant improvements in sleep efficiency and memory quotient(MQ)(P<0.05). Additionally, PSQI scores and alpha power in the superior and middle frontal gyri were significantly reduced(P<0.05). In the experimental group, the alpha band power at the right superior frontal gyrus(FC2 electrode) showed a positive correlation with daytime dysfunction (r=0.303,P=0.045), the memory quotient(MQ) was positively correlated with alpha band power, evidenced in the left superior frontal gyrus (AF3 electrode)(r=0.306,P=0.034); in the right middle frontal gyrus (F6 electrode)(r=0.300,P=0.038); in the right superior frontal gyrus (FC2 electrode)(r=0.301, P=0.038); and in the right middle frontal gyrus (FC4 electrode)(r=0.308,P=0.033).
Conclusion:rTMS combined with Tai Chi Chuan significantly improve sleep quality in elderly patients with sleep disorders. This improvement is achieved by reducing the increased frontal alpha power during memory maintenance, thereby effectively promoting the enhancement of memory function. |
