      摘要 目的:观察针刀疗法对腰椎间盘突出症根性神经痛大鼠下丘脑和脊髓中5-羟色胺、β-内啡肽含量的影响,探讨针刀疗法对腰椎间盘突出症根性神经痛干预作用的可能机制。 方法:将40只雄性SD大鼠随机分为正常组、假模型组、模型组、针刀组,共4组,每组10只。用热痛测试仪检测各组大鼠造模前后的缩爪潜伏期;HE染色观察L5神经根的病理学变化;造模后第8天分别取各组大鼠下丘脑和脊髓,采用双抗夹心ELISA方法检测下丘脑和脊髓中5-羟色胺、β-内啡肽的含量。 结果:模型组下丘脑和脊髓中5-羟色胺含量较正常组显著降低,差异有显著性意义(P<0.05),针刀组较模型组显著升高,差异有显著性意义(P<0.05);模型组β-内啡肽含量较正常组显著升高,差异有显著性意义(P<0.01),针刀组较模型组显著降低,差异有显著性意义(P<0.05)。 结论:针刀治疗可调节中枢疼痛相关递质5-羟色胺和β-内啡肽的合成和分泌,降低中枢痛觉传递和痛觉过敏,从而有效地发挥了镇痛作用。

Abstract:
      Abstract Objective: To observe effects of acupotomy on 5-hydroxytryptamine(5-HT) and β-endorphin(β-EP) in hypothalamus and spinal cord of rats with discogenic radiculalgia, so as to explore possible mechanism of acupotomy. Method: Forty male SD rats were randomly divided into four groups: normal group, sham model group, model group and acupotomy group, 10 rats in each group. Samples were taken on the 8th d after modeling. The hypothalamus and spinal cord were taken to observe the changes of 5-HT and β-EP by ELISA. Result: In model group, 5-HT in hypothalamus and spinal cord decreased significantly (P<0.05) compared with normal group, in acupotomy group those increased compared with model group (P<0.05); In model group, β-EP in hypothalamus and spinal cord increased significantly (P<0.05) compared with normal group, in acupotomy group those decreased compared with model group (P<0.05). Conclusion: Acupotomy had benign effects on regulating 5-HT and β-EP in hypothalamus and spinal cord of rats with lumbar disc herniation and reducing central algotransmission and hyperalgia, so as to provide analgesic effect.
